    Abstract
Obstruction of left anterior descending artery (LAD) due to the thrombosis or atherosclerotic plaques is the leading cause of death worldwide. Targeted delivery of drugs through micro- and nanoparticles is a very promising approach for developing new strategies in clot-busting or treating restenosis. In this work, we modelled the blood flow characteristics in a patient-specific reconstructed LAD artery by the fluid–solid interaction method and based on physiological boundary conditions. Next, we provided a Lagrangian description of micro- and nanoparticles dynamics in the blood flow considering their Brownian motion and the particle–particle interactions. Our results state that the number of...

    Abstract
Cardiovascular disease is one of the most rampant causes of death around the world and was deemed as a major illness in Middle and Old ages. Coronary artery disease, in particular, is a widespread cardiovascular malady entailing high mortality rates. Angiography is, more often than not, regarded as the best method for the diagnosis of coronary artery disease; on the other hand, it is associated with high costs and major side effects. Much research has, therefore, been conducted using machine learning and data mining so as to seek alternative modalities. Accordingly, we herein propose a highly accurate hybrid method for the diagnosis of coronary artery disease. As a matter of fact, the...

Simulation of LAD-LCX Bifurcation Hemodynamics of Coronary Artery Disease to Design an Effective Bypass Graft

, M.Sc. Thesis Sharif University of Technology Tajeddini Sarvestani, Farshad (Author) ; Firoozabadi, Bahar (Supervisor) ; Afshin, Hossein (Supervisor) ; Pakravan, Hossein Ali (Co-Supervisor)
    Abstract
In this study, three recent predictors of severity of coronary stenosis including fractional flow reserve (FFR), instantaneous wave free-ratio (iFR) and coronary flow reserve (CFR), are calculated for a number of patient-specific models of left coronary arteries. These patients are admitted to Tehran heart center. The main aim is presenting the optimum numerical and clinical detection technique to assess the severity of coronary stenosis. The effect of geometric features of stenosis is studied on the results of the FFR, iFR, and CFR in order to determine their accuracy. It is observed that CFR does not have a good estimate of the amount of disease. But, iFR, as a less risky test than FFR,...

Design an Analysis of a Heartbeat Motion Compensation Controller in a Robotic Coronary Artery Bypass Graft Surgery

, M.Sc. Thesis Sharif University of Technology Rahmati, Zahra (Author) ; Behzadipour, Saeed (Supervisor)
    Abstract
The Coronary artery bypass grafting surgery is one of the most complicated and invasive surgeries, in which the chest is cleaved up to 20 cm. Also, heart motion with 1-2Hz frequency bothers surgeons to perform on. This leads surgeons to get interested in minimally invasive robotic surgery methods. The idea is to synchronize the robot motion with heart, in order for surgeons to perceive it stationary. This needs 3 major stages: online taking of data, estimate the future heart motion, and control the robot. First studies employ visual seroving for control demand. Later studies, although differ in data collection methods, rely on classic error-based feedback controllers, such as PID. Fast...

A fuzzy expert system for prescribing atorvastatin optimum dose

    Abstract
Introduction: Today, Fuzzy expert systems have helped physicians in cases of uncertainty in medical decision making for prescribing drugs. Narrowing of coronary heart and reduction in heart blood flow are important causes of coronary heart disease (CHD). Control of blood lipid level is a routine treatment in CHD patients. But damage to liver is a common side effect of lipid lowering drug medication. Measurement of plasma Alanine aminotransferase level is considered as a marker of liver damage. The aim of this study was to determination of optimal dose of atorvastatin using Fuzzy expert system. Materials and Methods: in this study, Fuzzy expert system based on drug dose determination was...

Electrocardiographic ST-segment depression and exposure to traffic-related aerosols in elderly subjects with coronary artery disease

    , Article Environmental Health Perspectives ; Volume 119, Issue 2 , Feb , 2011 , Pages 196-202 ; 00916765 (ISSN) Delfino, R. J ; Gillen, D. L ; Tjoa, T ; Staimer, N ; Polidori, A ; Arhami, M ; Sioutas, C ; Longhurst, J ; Sharif University of Technology

    Abstract
Background: Air pollutants have not been associated with ambulatory electrocardiographic evidence of ST-segment depression ≥ 1 mm (probable cardiac ischemia). We previously found that markers of primary (combustion-related) organic aerosols and gases were positively associated with circulating biomarkers of inflammation and ambulatory blood pressure in the present cohort panel study of elderly subjects with coronary artery disease. Objectives: We specifically aimed to evaluate whether exposure markers of primary organic aerosols and ultrafine particles were more strongly associated with ST-segment depression of ≥ 1 mm than were secondary organic aerosols or PM 2.5 (particulate matter with...

Fsi simulation of a healthy coronary bifurcation for studying the mechanical stimuli of endothelial cells under different physiological conditions

    Abstract
Atherosclerosis is a world-spread and well-known disease. This disease strongly relates to the endothelial cells (ECs) function. Normally, the endothelial cells align in the flow direction in the atheroprotected sites; however, in the case of atheroprone sites these cells orient randomly. The mechanical stimuli such as wall shear stress and strains could determine the morphology and function of the endothelial cells. In the present study, we numerically simulated the left main coronary artery (LCA) and its branches to left anterior descending (LAD) and left circumflex coronary (LCX) artery using fluid-structure interaction (FSI) modeling. The results were presented as longitudinal and...

A validated reduced-order dynamic model of nitric oxide regulation in coronary arteries

    Abstract
Nitric Oxide (NO) provides myocardial oxygen demands of the heart during exercise and cardiac pacing and also prevents cardiovascular diseases such as atherosclerosis and platelet adhesion and aggregation. However, the direct in vivo measurement of NO in coronary arteries is still challenging. To address this matter, a mathematical model of dynamic changes of calcium and NO concentration in the coronary artery was developed for the first time. The model is able to simulate the effect of NO release in coronary arteries and its impact on the hemodynamics of the coronary arterial tree and also to investigate the vasodilation effects of arteries during cardiac pacing. For these purposes, flow...

Simulation of low density lipoprotein (LDL) permeation into multilayer coronary arterial wall: interactive effects of wall shear stress and fluid-structure interaction in hypertension

    , Article Journal of Biomechanics ; Volume 67 , 2018 , Pages 114-122 ; 00219290 (ISSN) Roustaei, M ; Nikmaneshi, M. R ; Firoozabadi, B ; Sharif University of Technology

    Abstract
Due to increased atherosclerosis-caused mortality, identification of its genesis and development is of great importance. Although, key factors of the origin of the disease is still unknown, it is widely believed that cholesterol particle penetration and accumulation in arterial wall is mainly responsible for further wall thickening and decreased rate of blood flow during a gradual progression. To date, various effective components are recognized whose simultaneous consideration would lead to a more accurate approximation of Low Density Lipoprotein (LDL) distribution within the wall. In this research, a multilayer Fluid-Structure Interaction (FSI) model is studied to simulate the penetration...

    Abstract
Coronary artery disease (CAD) is the prevalent reason of mortality all around the world. Targeting CAD, specifically atherosclerosis, with controlled delivery of micro and nanoparticles, as drug carriers, is a very proficient approach. In this work, a patient-specific and realistic model of an atherosclerotic plaque in the left anterior descending (LAD) artery was created by image-processing of CT-scan images and implementing a finite-element mesh. Next, a fluid-solid interaction simulation considering the physiological boundary conditions was conducted. By considering the simulated force fields and particle-particle interactions, the correlation between injected particles at each cardiac...
